Complex Care Management
Complex care management is simply a tailored service that is put together to ensure that all your complex health needs are understood and catered for. This will be created by experienced complex care providers and professionals.
For example, we have supported for individuals with a wide variety of challenging behaviour and health conditions. Part of our complex care management involves ensuring we work with all the community teams in place and family members who are part of the complex care package.
How We Administer Complex Care Management
- Creating a fully tailored intensive or long-term complex care plan for our client in the location they dwell.
- A dedicated senior case manager will attend all reviews, daily Huddles and assessments (with other professionals), implementing specialist intervention and assessments when required. Making sure our clients are receiving the right level and type of support.
- Ascertaining a single point of contact for each client with 24-hour-on-call support
from case managers and Specialist Care providers - Providing specialist care workers and the support of other health and social care professionals to
meet the client’s needs.
